Benign Appendiceal Mucocele: Safe Surgical Management. A Case Report and Literature Review
Abstract
Introducción: el mucocele apendicular es una entidad rara, presente en el 0,2 – 0,7 % de las apendicectomías y con frecuencia simula un cuadro de apendicitis aguda.
Objetivo: presentar un caso de mucocele apendicular con diagnóstico ecográfico inicial.
Presentación del caso: se presenta caso de un paciente masculino de 53 años con dolor en fosa ilíaca derecha de 72 horas, leucocitosis (14,500/µL) y ecografía abdominal que reveló una estructura quística no compresible adyacente al ciego. La laparotomía exploradora identificó un apéndice dilatado con contenido mucinoso, sin signos de malignidad. Se realizó Apendicectomía convencional, confirmándose histopatológicamente un mucocele apendicular benigno (adenoma mucinoso, CK20+). Este caso destaca la utilidad de la ecografía como primera línea diagnóstica en contextos con recursos limitados, mostrando sensibilidad del 85%; la seguridad del abordaje abierto; y la necesidad de seguimiento con ecografía anual, dada la posibilidad de recurrencia o malignización.
Conclusiones: el mucocele apendicular requiere un alto índice de sospecha basado en los hallazgos clínicos e imagenológicos atípicos. La correlación multidisciplinaria entre cirujano, radiólogo y patólogo es esencial para el manejo óptimo.
