Radiosurgery in cervicaluterine cancer prevention program in Pinar del Río

Abstract

Introduction: radiosurgery as a diagnosis and therapeutical means is used in Pinar del Río from November 2003, and the assessment of its effectiveness should be parmanent.
Objective: to evaluate the impact of the use of the radiosurgery cone on Pinar del Río from 2004-12.
Material and methods: An observation, descriptive and retrospective study in the Province Department of Cervix Pathology at Abel Santamaria Cuadrado University General Hospital (patients from the diagnosis program or not). Methods of descriptive statistics were used, and the Pearson correlation coefficient, ANOVA and Student´s T test.
Results: 15,836 women attended, out of which 6,695 (42.3%) received readiosurgey, 774 by year average, from all of the municipalities and none under anaesthsia. 49.5% of the program (38,0±8,7 years, CI 95%: 36,7_39.4 yearsaños), the rest was not in the program (34.3±10.9 years, CI 95%: 33,1-35,5 years). The most frequent diagnosis was cervical intraepithelial neoplasia (CIN1), 44 invasive epidermoid carcinoma in stages Ib or higher and 3 endocervical adenocarcinomas. The positive margins (5.1%) predominated in the patients with carcinoma in situ and the natural history of two cases was invasive carcinoma. 17.8% presented residual disease.
Conclusions: ambulatory radiosurgery is effective when 100% curable lesions are diagnosed. The better state of the surgical piece has been noticeably achieved. Lesser cases with positive margins, and therefore less possibility of residual lesion. There is a positive impact on the Cervicauterine Cancer Early Detection Program.
