La isquemia miocardica silente. Criterios actuales. Silent myocardial ischemia. Current criteria
Abstract
Esta revisión describe los criterios actuales para la definición, clasificación, diagnóstico y tratamiento de la isquemia miocárdica silente (IMS) así como su significado pronóstico en aquellos pacientes portadores de ella teniendo en cuenta las sugerencias del American College of Cardiology y la American Heart Association (ACC/AHA), se expone la importancia de la identificación, evaluación y prevención de los factores de riesgo más importantes que favorecen la aparición de la isquemia miocárdica, así como los más recientes adelantos en el manejo de esta entidad tanto desde el punto de vista farmacológico como no farmacológico (revascularización coronaria con angioplastía o cirugía). Conclusiones: La modificación, eliminación o control de los principales factores de riesgo cardiovascular susceptibles de manipulación y de las posibles causas desencadenantes de IMS una vez que esta ha sido identificada, continúan siendo los objetivos principales a alcanzar para disminuir la ¨carga isquémica total¨ y mejorar el pronóstico a largo plazo de estos pacientes. La elección del fármaco debe ser discriminada mediante criterios clínicos o derivados de exploraciones como prueba de esfuerzo, monitorización ambulatoria de holter o talio-201 de esfuerzo entre otros exámenes que se precisan de forma resumida en esta revisión.

ABSTRACT
The report describes the current criteria to the definition, classification, diagnosis and treatment of Silent Myocardial Ischemia (SMI) as well as the prognosis in patients suffering from SMI taking into account the suggestions of the American College or Cardiology (ACC) and the American Heart Association (AHA). The importance of the identification, assessment and prevention of the risk factors wish favors the onset of the MI are analysed and the current advantages on the management of this entity from the pharmacological and nonpharmacological view point (coronary revascularization with angioplasty and surgery). Conclusions: once SMI has been identified the mortification, elimination or control of the principal manageable cardiovascular risk factors and the possible causes of SMI onset continue being the principal aims to decrease the "total ischemic load" and improve the long - term prognosis of these patients. Drug's choice should be distinguished taking into account clinical criteria or by means of exercise test, ambulatory exercise monitoring by holter or talio 201 among other required examinations summarised on this report.
