Carcinoma renal enmascarado por sepsis en una gestante adolescente / Renal Cell Carcinoma masked by sepsis in a adolescent pregnant
Abstract
Joven femenina de raza negra, 18 años de edad y 12 semanas de embarazo, debuta con manifestaciones sépticas, fiebre alta, escalofríos y dolor lumbar, con diagnostico clínico e imaginológico de abceso renal derecho; es ingresada en el Servicio de Ginecología y Obstetricia del Hospital Docente "Abel Santamaría" de Pinar del Río, con tratamiento sintomático y amplia cobertura antibiótica, con evolución clínica satisfactoria, de forma contradictoria con la Imaginología y los parámetros de laboratorios. Se decide interrupción del embarazo, posteriormente se realiza nefrectomía derecha y la biopsia reportó carcinoma de células renales, clasificado Etapa Clínica Ib. La evolución post operatoria fue satisfactoria. Se inicia tratamiento con inmunoterapia: Interferón alfa 2b humano recombinante (HEBERON) por factores pronósticos desfavorables. Actualmente la paciente se mantiene asintomática.

ABSTRACT
An eighteen black female patient with 12 weeks pregnancy presenting septic symptoms: high fever, shivers, lumbar pain and with a clinical and imaging diagnosis of a right renal abscess was admitted at Gynaecological and Obstetric Service in "Abel Santamaria" University Hospital, Pinar del Rio, covering her with antibiotics and symptomatic treatment, having a satisfactory clinical progress on the contrary to Imaging and laboratory parameters. Pregnancy was interrupted and a right nephrotomy was performed; the biopsy showed a renal cell carcinoma classified in the Clinical Stage (Ib). The patient presented a satisfactory Post-operative progress. Immunotherapy started: Recombinant Human Interferon á 2b (HEBERON) due to unfavourable prognoses. Currently, the patient is asymptomatic.
