Psychiatric alterations related to gastric trichobezoar. Case report

Introduction: Trichobezoar is a type of phytobezoar composed of hair, which is trapped by the gastric folds and prevents its mobilization and excretion, causing it to accumulate over time and the mass to increase in size. It is uncommon and usually occurs in the stomach or small intestine, causing symptoms typical of the gastrointestinal tract.
Objective: To report a case of gastric trichobezoar and its relationship with psychiatric disorders by reviewing the clinical history.
Presentation of the case: 15-year-old adolescent presented to the doctor for a clinical picture of 10 days duration, with diarrheal stools and vomiting, after three days the picture was exacerbated, adding to the symptoms nausea that reached vomiting, diffuse cramping abdominal pain of great intensity in the epigastrium, accompanied by diaphoresis and profuse sweating. On physical examination, abdomen was distended, globular, painful on superficial palpation, decreased RHA present. Therefore, an interconsultation is made to general surgery where an exploratory laparotomy is prescribed where the finding and extraction of a trichobezoar in the stomach is performed due to CIE - 10 F 633 trichotillomania that maintains follow-up with psychology.
Conclusions: It was evidenced the remarkable relationship between psychological alterations, female gender and the presentation of the disease called Trichobezoar.
