Fibrous dysplasia of paranasal sinuses. Case Presentation

Introduction: Fibrous dysplasia is a developmental defect characterized by focal sequestration of normal osteoblastic activity of bone in the absence of maturation of its components. The result is a lesion consisting of fibrous stroma with islets of undifferentiated bone.
Objective: to present an atypical clinical case of fibrous dysplasia of the paranasal sinuses.
Case presentation: female patient, mixed race, 50 years old, with a history of ophthalmologic surgery due to volume increase in the left upper eyelid, for which surgical treatment was decided, where a lesion of hard aspect was observed with a diagnostic impression of bone tumor and histologic diagnosis of osteoid osteoma of the eye. Referred to the Otorhinolaryngology Department of the Clinical and Surgical University Hospital Comandante Manuel Fajardo in Havana, endoscopic surgery of perinasal sinuses was performed by conservative exeresis due to tumor expansion and orbital decompression to reduce ocular and nasosinusal manifestations. Histologically the presence of fibrous dysplasia of frontal sinus and ethmoidal cells was determined. Postoperative antibiotic and corticosteroid treatment is indicated. The patient is evaluated every six months, without complications, oncologic degeneration or other alterations.
Conclusions: Fibrous dysplasia in paranasal sinuses is a rare pathology that should be taken into account in adult patients with slowly progressive proptosis and poor visual capacity. Although recurrence of the lesion is not frequent, early histopathologic diagnosis and surgical removal of the lesion is important to avoid complications and alleviate present symptomatology.
