Introduction: seborrheic keratoses are a common type of benign tumor of epithelial lineage. They can appear in any part of the body, but are more common in keratinized skin and rarely in mucous membranes.
Objective: to describe a case of a patient with seborrheic keratosis treated with a modified Z-flap O-flap.
Case presentation: we present a case of a 76-year-old male patient, white, from a rural area, with health history, agricultural worker, who presented a lesion in the scalp region on the right side, which was increasing in size and changing its appearance in seven months of evolution. He was surgically treated with a double rotation advancement flap (O in Z) with a modification and the histological study showed seborrheic keratosis.
Conclusions: We presented a patient with the presence of a seborrheic keratosis. This is a great experience, since no lesion with these characteristics had been observed in the Maxillofacial Surgery Department.
