Quiste de mesenterio gigante: reporte de un caso en el Hospital Provincial Estatal de Bengo, República Popular de Angola / Giant cyst of the mesentery: a case report at "Bengo" State Provincial Hospital, Popular Republic of Angola
Abstract
El quiste del mesenterio se subdivide según su origen, en tumores linfáticos, mesoteliales, urogenitales, dermoides, entéricos y seudoquistes. El quiste de mesenterio es una entidad poco frecuente de escasa sintomatología que se presenta según la clínica como una obstrucción intestinal, hidronefrosis por compresión y abdomen aguda. Se trata de un enfermo masculino de 7 años de edad, que al nacer ya se le palpaba una masa abdominal, es recibido en el Hospital Provincial Estatal de Bengo (República Popular de Angola) por presentar un aumento marcado del abdomen que le ocasionaba molestias abdominales. Durante el examen físico se constató que la masa abdominal palpable renitente ocupaba casi todo el abdomen hasta la parte más baja del hipogastrio de aspecto quístico y ensanchamiento de la base del tórax. Fue realizado ultrasonido abdominal, el cual confirmó una imagen quística gigante. Se procedió al tratamiento quirúrgico extirpándose el quiste de 27 cm de diámetro, 4400g de peso a nivel de mesenterio, con contenido de unos 5 litros de líquido transparente, sin provocar compromiso vascular de las asas intestinales; los resultados de la anatomía patológica informan mesoteliona quístico benigno. Su evolución postoperatoria fue favorable. Por la baja incidencia de la enfermedad y el tamaño del quiste se reportó el caso.

ABSTRACT
The cyst of the mesentery is subdivided according to its origin as: lymphatic, mesothelial, urogenital, dermoid, enteric and pseudocystic tumors. The cyst of the mesentery is an uncommon entity, symptoms are rare and it is clinically present as an intestinal obstruction, hydronephrosis and acute abdomen. A 7 year-old male patient who at birth an abdominal mass was palpated, the patient admitted at "Bengo" State Provincial Hospital, Angola Republic presented a marked volume of the abdomen which caused abdominal upsets; verifying during physical examination the renitence of an abdominal mass which occupied most of the abdomen up to the lowest part of the hypogastrium with a cystic aspect, widening to the base of the thorax. An abdominal sonography was performed, it confirmed a giant cystic. A cyst of 27 cm of diameter, weighting 4400g at the level of the mesentery containing 5 liters of a transparent fluid was removed without vascular damages of the intestinal masses; the results of the pathology reported a benign cystic mesothelioma. Postoperative evolution was favorable. Due to the low incidence of this entity and the size of the cyst the case was reported.
