Introduction: Patient safety is a fundamental principle of healthcare, and is now recognized as a broad and growing challenge for global public health. Objectives: show the common causes of harm to patients, expose the essential actions for patient safety and suggest the implementation of actions to reduce and prevent adverse events derived from medical care. Methods: a review of the international literature was carried out and the experience of 6 Cuban intensives was compiled, who collaborated in care in Mexico and Italia hospitals. Results: the most common causes of harm to patients were identified and the eight essential actions for patient safety were presented. Conclusions: the main suggestions to achieve sustainable and significant improvements in health care safety are the development of institutional policies clear, the leadership capacity of managers, the existence of data records that allow risk assessment, the training and systematic evaluation of professionals and the effective participation of patients and families in the care process.
