Introduction: agranulocytosis is a rare pathological condition in clinical practice, a potentially serious cause of leukopenia. It may be of central or peripheral origin and it is characterized by the high frequency of acute infections in sick people. The vast majority of severe, acute, isolated, and reversible neutropenia are of drug origin.Case Report: a 49-year-old male patient, presenting high respiratory symptoms due to coryza, sneezing and fever for several days of evolution, receiving symptomatic treatment with dipyrone; the fever became higher and persistent affecting his general condition. He came to the health services where the diagnosis of agranulocytosis associated with dipyrone was made. Given its severity he was admitted to an intensive care unit in isolation with broad spectrum antibiotic-therapy, granulocytic colony stimulating factor, and eventually died.
Conclusion: this case alerts us concerning the importance of early detection of leukopenia due to dipyrone, although the patient received adequate treatment for his condition, the evolution was unfavourable and he died. This shows the serious prognosis of this disease.
