Criptococosis del sistema nervioso central. Presentación de un caso. Cryptococcosis of the Central Nervous System. A case report

Resumen

Se presento el caso de un paciente masculino, blanco de 23 años de eda, que ingresó en el Hospital Provincial Clínico Quirúrgico de Pinar del Río Abel Santamaría, con un cuadro de fiebre, cefalea y malestar general, apareciendo días más tarde rigidez mucal y otros síntomas y signos de afectación del sistema nervioso central. El diagnóstico de meningoencefalitis fue hecho por el estudio citoquímico del líquido cefalorraquídeo y se confirmó la presencia de criptococcus neoformans con la coloración de tinta china y el cultivo micológico del líquido cefalorraquídeo. El tratamiento con anfotericin B por vía sistémica e intratecal fue impuesto de inmediato y prolongado durante seis semanas, posibilitando la curación del paciente, el cual quedó con secuelas neurológicas que lo hacen mantenerse aún bajo tratamiento de rehabilitación.

ABSTRACT

A 23 year old white male patient having fever, headache and a general malaise was admitted at Abel Santamaria General Hospital. After few days of admission, symptoms like nuchal rigidity and affections of the Central Nervous System (CNS) appeared in the patient. A citochemical study of cerebrospinal fluid resulted in a meningoencephalitis diagnosis; the presence of criptococcus neoformans was confirmed by India-ink staining and mycological culture of cerebrospinal fluid. The patient was inmediately treated with amphotericin B by systemic and intrathecal via for six weeks. This treatment provided patient's healing, although neurological sequelae remained; thus, keeping him under a rehabilitation treatment.
